POP - Progesterone Only Pill Review Form

·         You require a review of your progesterone only contraceptive pill (POP) prior to being issued with a further supply. Progesterone only contraceptive pills include: Desogestrel 75 micrograms/Cerazette/Cerelle/Norethisterone 350 micrograms/Noriday/Hana/Lovima. If your pill is not listed then do not continue with this form - please ocntact reception for a Practice Nurse appointment.

      This form is not for renewal of a prescription for the combined pill (tradionally taken 21 days on follwed by 7 days off)  for review of the combined oral contraceptive pill please make an appointment with the Practice Nurse 

          We now operating an online request system for review and renewal of this medication. This is to ensure an uninterrupted supply of your medication, and to avoid unnecessary appointments with the nurse of GP.

·         If you would like to continue on this method of contraception and do not feel that you need to speak to a GP or nurse, please answer the following questions so that your prescription can be renewed if appropriate.
