If at any point you are needing to hand in any samples that have been requested from your GP then these would need to be brought to the surgery before 3pm.

We would kindly ask if you could make sure your sample pot has the following:

  • Name                                        
  • Date of Birth
  • Date sample taken                                             
  • Time sample taken

If the samples do not have all the above information on them correctly then we unfortunately have to discard of these.

It is very important that we ask when you hand samples in to NOT leave them lying on the reception desk and wait to speak/hand them to a receptionist as they will pass the sample on for appropriate testing and they will need you to fill out a small form which can be found below.

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Prescriptions for taking medication abroad
  • Private sick notes (less than 7 days off work)
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
